What is Accutane?


Accutane (Isotretinoin) is a medication for severe acne created by Hoffmann-LaRoche Inc. (Roche). It is a vitamin A derivative (13-cis-retinoic acid) that is administered orally in pill form, normally for 15-20 weeks (3.5-4.5 months).


The drug, approved by the U.S. Food and Drug Administration (FDA) in 1982, proved to be a miracle for some, offering blemish-free skin to those who had given up after trying many other treatments.


It was originally prescribed for people with severe acne that did not respond to other treatments, but has gained in popularity in the past 25 years and is prescribed more and more frequently for less severe acne.


Accutane works by reducing the size and output of the sebaceous glands (skin oil glands) by 35% to 58% and thus stops excess oil production by around 80%. Acne bacteria (P. acnes) live in skin oil. Since oil is dramatically reduced, so is the amount of acne bacteria in the skin.


Accutane also slows down how fast the skin produces skin cells inside the pore, which helps pores from becoming clogged in the first place. Isotrteinoin also normalizes your keratinization (the shedding of dead skin cells), improves fat metabolism and reduces inflammation. It is the closest thing we have to for a “miracle” drug to cure acne.


But Isotretinoin does come with a host of possible side effects. Before starting Isotretinoin treatment, you should understand clearly what these side effects are.


The problem with Accutane is it damages your skins sebum glands and liver function in the process. By the time you stop taking the drug, your skin oil glands will return to function but your liver will not be able to properly balance your hormones and prevent the over production of sebum.


Because Isotretinoin shrinks the sebaceous glands, nearly everyone who takes it will experience dry skin to some degree. Your skin may flake or peel, your lips may get chapped, and your eyes and inside the nose might feel dry.

Isotretinoin can cause your skin to become more sensitive to the sun, or photosensitivity. You’ll be more susceptible to sun burn and sun damage. Other side effects associate with the use of Isotretinoin are thinning hair, decreased night vision, fatigue, nausea and vomiting, headaches, bone and joint pain, diarrhoea or rectal bleeding, severe chest or abdominal pain and difficult or painful swallowing.


Probably, the two most widely known side effects of Accutane treatment are its potential to cause severe birth defects if taken by pregnant women or women that became pregnant while taking the drug and the suggested link between the intake of Accutane and the changes in mood, severe depression and suicidal thoughts.

Since, Isotretinoin can be harmful to the fetus, it must never be used during pregnancy and women should avoid pregnancy for at least a month after the use of Accutane. Even one dose of the drug while pregnant can cause severe birth defects. Additionally, because of its severe side effects, nursing mothers must avoid the drug completely.

Psychiatric problems have also been linked to Accutane use. From 1982 to May 2000, the U.S. Food and Drug Administration (FDA) received hundreds of reports linking isotretinoin use to depression, including 37 suicides, 110 hospitalizations for depression or suicidal behavior, and 284 cases of non-hospitalized depression.


Roche responded to the reports of depression and suicide in 1998 by adding a warning to the medication’s label stating that Accutane could cause psychiatric disorders. This prompts the FDA to issue a warning that Accutane may cause depression, psychosis, thoughts of suicide, and suicide.
